Two kinds of difference
Text changes come from the words in each file, so edits are listed as before and after. Visual changes come from comparing the rendered pages pixel by pixel, which also catches changes to images, lines and stamps.
Check what an editor really changed
When someone sends back an edited PDF, compare it with the one you sent. Anything they changed shows up, including small edits that are easy to miss.

Visual changes are marked on scans too. Text changes need real text, so run both files through OCR PDF first for word level results.
+What if pages were inserted?
Pages are compared in order, so an inserted page shifts everything after it. For those cases, extract the matching pages first.
+How many pages can I compare?
Up to 60 pages per file.
+Are my files kept on your server?
